What insurances do you accept?
We accept most Blue Cross Blue Shield plans (excluding Blue Home), Cigna, Medicare, and Aetna PPO or employer sponsored plans. If you have questions regarding your coverage, contact your insurance provider for more information.
What if my insurance is out-of-network?
You will be responsible for paying our self-pay rates at the time of your appointment. If you would like to independently submit your claims for any out-of-network coverage your plan may offer, we can provide you with payment receipts and other information needed. To find out our self-pay rates, contact us.
Will I need a referral for physical therapy?
While Team Care does not require a referral for treatment, some insurance plans require it, including Medicare. It is best to confirm with your insurance provider if you are unsure.
What should I bring with me to my appointment?
For your initial evaluation, we will need to scan your ID and insurance card(s) upon arrival. Before your appointment, please complete the short survey in your email. If you have questions or cannot locate the survey, contact us.
Where are you located?
Our address is 140 Iowa Lane, Suite 104, Cary, NC 27511. We are conveniently located off of Kilmayne Drive, near the intersection of Kildaire Farm Road and SW Maynard Road, in the Kilmayne Office Center. We are close to the Raleigh, Durham, Apex, and the RDU International Airport.
